This Terms of Service Agreement sets forth the terms and conditions governing the provision of services by Forbes.health. By working with us and using our services, you accept the following terms.

The Practitioner offers deeply personalised advice based on functional medicine, aimed at improving your overall health and well-being. Our approach includes a 360-degree evaluation of your lifestyle, nutrition, stress management, and environmental factors.

Expertise in Functional Medicine

The Practitioner specialises in functional medicine, with extensive knowledge of personalised healthcare and preventive medicine. Functional medicine focuses on identifying and addressing the root causes of health concerns through personalised, evidence-informed strategies. While we do not provide medical diagnoses, we offer comprehensive insights based on scientifically supported methods and clinical experience.

Collaboration with Medical Professionals

While our services are not intended to replace medical advice, we collaborate with other healthcare professionals to support you in optimising your health. We encourage you to make medical decisions in consultation with your physician.

Accuracy of Information

The Practitioner strives to provide accurate, reliable, and scientifically based information. While we make every effort to offer the highest quality guidance, we recommend seeking additional professional advice as needed.

Limitation of Liability

The Practitioner shall not be held liable for any losses incurred by clients arising from reliance upon the proffered advice.

Personalised Health Plan

We help you create a customised health plan that takes into account your unique genetic makeup, lifestyle habits, and health goals.

Continuous Support

Through our online client portal, we provide ongoing support and guidance, including regular check-ins, progress assessments, and the ability to ask questions as needed.

Practitioner Assignment and Continuity

Forbes.health may assign or change practitioners during the course of a programme where reasonably necessary to ensure continuity, availability, operational requirements, or alignment with the client's needs and health goals.

No Guarantee of Results

The Practitioner aims to support and guide you toward better health, but results may vary depending on individual circumstances. Your personal commitment, consistency, and lifestyle choices play a crucial role in the effectiveness of the recommendations.

HIPAA Coverage

The Practitioner does not qualify as a covered entity or business associate under the Health Insurance Portability and Accountability Act (HIPAA).

No Diagnosis or CPT Codes

The Practitioner does not provide formal medical diagnoses or Current Procedural Terminology codes for insurance purposes. Instead, the focus lies on identifying underlying imbalances in the body to support your overall health through functional and lifestyle-based approaches.

Costs

Please be aware that the expenses for the services provided by the Practitioner and laboratory tests are not covered or reimbursed by any private medical insurance.

Programme Upgrades

A purchased programme can be upgraded to a higher-level programme during the course of the programme upon request and payment of the price difference. However, once upgraded, it is not possible to downgrade to a lower-level programme or receive a refund for any price difference. After completing a full programme, the Practitioner can advise on a suitable follow-up.

Completion of Home Test Kits

To maintain continuity and accuracy in the personalised health programme, clients must complete and return all home-delivered laboratory test kits within 10 calendar days of receiving the kit. Failure to complete the tests within this period may delay the interpretation process and affect the scheduling of consultations or follow-up assessments.

Scope of Services

The Practitioner obtains a series of laboratory test results on your behalf from Certified Laboratories in the United States, Canada, Europe, and the UK, and provides you with a comprehensive, 360-degree or nutrition-related interpretation of those test results. These test results can be exclusively used by you as an educational tool for your personal health purposes.

The Practitioner offers round-the-clock accessibility through their Client Portal (Practice Better) for addressing brief inquiries and providing support. Please note that 24-Hour Access is limited to messaging and does not include extended consultation sessions. If clients require a consultation with the Practitioner, they must schedule one separately at a rate of 250 Euros per hour. Consultations cancelled with less than 24 hours' notice may be charged in full.

The Practitioner uses test reports to identify patterns and potential imbalances in the body that may be impacting your overall wellbeing. These insights are used to support personalised, functional health recommendations. The information provided is intended as an educational and supportive tool and does not replace a consultation with a licensed medical professional. While we aim to empower you with a deeper understanding of your health, the identification or treatment of medical conditions or diseases falls outside the scope of our services.

The Practitioner may provide information about products that we believe might benefit you, but such information should not be construed as an endorsement or recommendation for disease treatment. Some products may require a prescription, but we do not dispense or prescribe any prescription products. The Practitioner does not warrant or guarantee the accuracy of the laboratory test results they receive. They provide no warranty regarding the quality or effectiveness of any interpretation, apparatus, advice, treatment, or product, and are not liable for any physical or mental injury or adverse side effects arising from the use of such interpretation, apparatus, advice, treatment, or product. While we make every effort to ensure the information shared via websites, videos, publications, emails, or other material is accurate and up to date, we cannot guarantee that all information is free from errors or omissions. Limitation of Liability

They are not accountable for changes in laboratory names and prices beyond their control, nor for any injury or loss resulting from your chosen product or treatment. They are not liable for failing to identify any medical condition or disease, as this is not the purpose of their services. It is the client's responsibility to coordinate and organise the shipping of completed human test samples via their chosen postal method. The buyer is responsible for paying any applicable VAT for importing products, as well as all import-related costs.
